New Patient Paperwork
You have the option of completing the new patient paperwork prior to your first appointment to ensure an expedient visit. Find the new patient form below.
It can be easy to forget when you scheduled a dental appointment but now we're able to remind you with an Appointment Reminder via email or text message. Please print, sign, and bring this form with you on your next appointment. We'll do the rest!
Once you are ready, we'll perform a thorough exam, check for issues with your teeth or structure of your mouth, and work with you to create a treatment plan you're comfortable with. We’re here to listen to you and provide the personalized care you deserve, so please share your questions and preferences with us at any time.
Please arrive 15 minutes before your appointment time. This will allow us to check all your paperwork so you can get the most out of your time with your dental team. If you're running late, just give us a call to let us know.
Can paperwork be completed in-office instead of online?
Completing and submitting your paperwork online allows us to optimize your experience. If you do need to fill out paperwork in the office, please arrive a little earlier to do so to help us keep you on schedule.
What if I need to cancel?
If you are unable to make your appointment, please let us know at least 48 hours before your appointment so we can offer the time slot to another patient.
